Question: My brother woke up with shakiness, weakness, light headedness and just feels like he is going to die. he also has had swollen feet for the last 24 hrs. He is 53 years old and with high blood pressure treated with meds.

Welcome to HCM
As I can understand your brothers complaints are
He woke up with shakiness, weakness, light headedness
Swelling over legs since 24 hrs
And he is patient of high blood pressure and on meds.

I am suspecting
Cardiac Ischemia attack, night terror(bad dream), panic attack

My questions for you
1) was there associated chest pain, breathlessness, sweating, vomiting?
2) was it such first episode, how long did I it lasted? What did I he do to relieve it.
3) details of his current medications, with duration of treatment?
4) did he had any bad dream?

My advice for you-
1) consult a local cardiologist /physician immediately, as we cannot take a cardiac attack lightly.
2) urgent ECG and 2DEcho, to check for cardiac abnormalities and function. Ventricular failure is suspected considering swelling on legs.
3) cpk mb investigations to rule out cardiac ischemia attack.
