Brief Answer:
Yes, you need to be concerned

Detailed Answer:
Hello,
Your current symptoms are likely due to elevated blood pressure which is quite high which can leads to multiple problems. So it needs to be controlled.
You need to on medicines like Amlodipine telmisartan combination (Telma AM 40/5) once a day, which you can get prescribed from local doctor. Have low salt diet and monitor blood pressure regularly thrice a day for one week then once or twice a week and bp should not be persistently more than 140/90 mmhg. You should also have a healthy lifestyle like avoiding fatty, oily and high calorie diet.
Regular exercises like brisk walking according your capacity atleast 30 min a day and 5 days a week. Lots of green leafy vegetables, fruits, fish once or twice a week, avoid meat. Get your lipid profile and sugars tested once.
For your atrial fibrillation, you need to be on blood thinners like dabigatran or rivaroxaban in order to reduce risk of stroke.
Hope this helps you and get back if you have any doubts.
