Brief Answer:
Middle ear effusion.

Detailed Answer:
Hi,

Thank you for your query.

1. It seems that the cough due to the exertion has push mucus up the Eustachian Tube and into the ear.

2. If the Eustachian Tube Dysfunction (ETD) secondary to your chronic sinusitis is preventing the middle ear from draining or has caused a pressure change in the middle ear, you will hear gurgling sounds in the ear.

3. These conditions may also cause dizziness. This should settle down soon and it is important to prevent any falls.

4. An ear drum examination, a Tympanogram (Impedance Audiogram) and balance testing will confirm the diagnosis.

5. In the meanwhile, an anti-allergic mucolytic, steam inhalation, OTC nasal decongestant drops (such as 0.1% Xylometazoline) will give relief.

I hope that I have answered your query. If you have any more questions I will be available to answer them.

Regards.
