
What Causes Ear Pain While Suffering From Common Cold?

Hello Doctor - Thanks for taking sometime today to review and for helping me.
I have cold with runny nose and bleeding from one of the nose, Feels pain in the throat and also feel like ears are closed. once a while complete nose will be drying.
Is it because of heat in the body? Also there is no bowel movements since two days.

After going through the history, I do feel, you could be suffering from upper respiratory tract infection, probably of viral origin. Do not worry! With appropriate measures this can be taken care of. Bleeding form the nose is due to repeated rubbing of the nose.
My advice to you is:
1. Drink frequent sips of warm water. Adequate hydration reduces the throat irritation, sense of heat in the body and nasal bleed.
2. Do steam inhalation 2-3 times a day - Steam relieves the nasal congestion as well as ear block.
3. Antipyretics such as paracetamol can relieve your body ache and throat discomfort.
4. Try taking over the counter oral decongestants and saline nasal spray.

I am Dr Mittal.
I have read your message.
I think I can help you.
I will first summarize what I have understood from your message.
You had runny nose with some bleeding. There was throat pain with ear closure. And you had constipation. It seems that after the treatment, you have had partial recovery- throat issues, ear symptoms, and nasal bleeding persist.
I think this is a partially treated upper respiratory tract infection with possible sinusitis.
I think you should know what is happening, because that will help you understand the treatment.
Here is how it happens. There is nasal congestion, due to infection. The nasal cavity and throat are a continued structure. So the infection affects the throat as well, causing pain and irritation-soar throat possibly. The ear opens into the throat. The infection causes the opening to be blocked. The air in the ear gets absorbed by the body, causing the ear blockage and pain. The nose bleed can be due to the nasal congestion/picking of nose/blowing of nose (for nasal congestion)/ or heat of the environment. The infection is probably also causing fever.
I would suggest-
1. Betadine gargles 4 times a day (stop the saline gargles). This will help clear the throat congestion. Steam inhalation (preferably with Vicks, 4 times a day).
2. Take plenty of fluids. At least 4-5 liters per day is needed.
3. Take Nasal Drops Nasivion (if there is nasal congestion) 4 times a day, in either nostrils.
4. Avoid blowing your nose.
5. Apply ice immediately if you have nasal bleed actively. Else apply vaseline cream at the nasal bleed site. It will help the damaged skin/mucosa heal faster, and therefore reduce the bleeding.
6. Take a course of multivitamins- any brand that you have available can do. Once a day is enough. Try Revital once a day after food.
7. A short course of steroids may be useful in a very low dose. It will help clear the congestion of the throat, and may relieve the ear pain as well. A small dose of dexamethasone 0.5 mg once a day should be enough.
8. Antibiotics would be needed. I would ideally have suggested meeting a local doctor for the antibiotic. Because infective agents in different areas respond to different antibiotics. Still if needed, I think a course of levofloxacin 500 mg once a day should suffice.
9. Tab Pantocid 40 once a day on empty stomach, half hour prior to food.
10. Tab Paracetamol 500 mg, sos if you develop fever.
Please do note, the entire course of treatment would be 7 days.
I would also suggest the following tests-
1. a CBC, PT/INR, aPTT, FBS.
2. a Throat Swab Culture.
3. X ray PNS Waters view, Chest X Ray (PA view).
The throat swab will have to be done by the local lab technician, or a nurse.
