What Causes Elevated SGOT And SGPT Levels?

Currently, SGOT is 90, and SGPT is 223. SGOT and SGPT have remained high for 6 weeks. Albumin is 5.5 and Ferritin is 758. I am working on scheduling a biopsy.
I was very sick (105F fever, vomiting) for several days, which is what led to the original CT scan. I have tested negative for hepatitis, and aside from slight hypertension, all other levels are normal. What is this likely to be?
Fatty liver disease
Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.
Both your SGOT and SGPT are significantly high.
Though there are many different causes for raised liver enzymes, nonalcoholic fatty liver disease is the most common cause.
Your BMI of 36 indicates you are obese, Obesity is the most common risk factor for fatty liver disease.
You have ruled out other causes viral infection.
Tests for metabolic liver disease and autoimmune liver disease are to be done.
Though splenomegaly may indicate portal hypertension which is a complication of cirrhosis of liver, your CT scan didnt show shrunken liver with surface nodularity which are features of cirrhosis.
So all you have is nonalcoholic fatty liver disease.
In addition to the tests I have mentioned above, you can get a liver biopsy done as already planned.
Let me know if you have any more questions.
Regards,
Dr.R.K.

Ferritin more than 1000 usually causes liver issues
Detailed Answer:
Hi,
Thanks for getting back to me with more questions.
Ferritin more than 1000 is usually associated with liver disease due to hemo hemochromatosis.
Your ferritin is not that high.
Since you have planned liver biopsy let's wait for that as biopsy tissue examination will provide information about the ferritin related liver disease aspect also.
Regards,
Dr.R.K
