What Causes Elevated SGPT, SGOT And GGT Levels?
Question: Hi, i'm 43 male ..in February 2016 i did a liver and kidney panel (regular checkup) all results were normal and within range. two days ago i felt severe and continuous pain in my stomach, i went to hospital and the ER did an USG Whole Abdomen + CBC (complete blood count), EDTA ..the findings are as following:
USG Whole Abdomen:
1. Liver Enlarged 17.8 cm with normal echogenicity, No focal lession is seen.
2. Gallbladder is ok.
3. Pancreas is ok.
4. Spleen normal.
5. Kidneys normal size.
6. Urinary bladder normal.
7. Prostate is mildly enlarged.
Impression:
1. Mild hepatomegaly.
2. Mild Prostatomegaly (BPH grade 1)
CBC (complete blood count), EDTA:
All results are within normal range, except Lymphocytes (result 42.5 ...reference range 20 - 40), Eosinophils (result 6.6 .... reference range 1 - 6).
The next day i went to dr lal path and did Liver panel test which the result shocked me:
1. AST (SGOT) result (258) reference range <50.
2. ALT (SGPT) result (404) reference range <50.
3. GGTP result (446) reference range <55.
4. Alkaline Phospatase (ALP), result (129) reference range 30 - 120.
Medicine that i use on daily basis:
1. GNC Triple strength fish oil.
2. GNC mega men multivitamin.
3. Lisinopril 10 mg for blood pressure.
4. UbiActiv QH Ubiquinol.
5. liv 52.ds
6. Milk Thistle and dandelion.
i'm also a moderate to heavy drinker, also worth mentioning that i preformed a bariatric surgery two years ago which i took out 75% of my stomach and lost over 55 KG since two years.
your advice and assistance is much appreciated.
forgot to mention, i also use three types of gensing (korean, XXXXXXX Siberian) also i'm currently quiting smoking for 17 days now and use champix (varenicline) to aid in quiting smoking.
Sorry, forgot the operation medical name i lost the name whether it's sleeve gastrectomy or bariatric surgery (i just took 75% of stomach).
I keep forgetting, also did Hepatitis C & B ..results are normal (non responsive)

Elevation in SGOT, SGPT suggests that there is injury to liver cells. GGT elevation suggest that this is due to alcohol intake. Since you are a moderate or heavy drinker it is all due to alcohol intake leading to alcohol hepatitis. Ultrasound abdomen also showed hepatomegaly
So I will suggest you to quit alcohol intake completely
Few medicines which are helpful are ursodeoxycholic acid 300mg thrice a day. this is to be taken till sgot and sgpt becomes normal. Get your sgot and sgpt check twice in a month.

There are few stages of progression of alchol related liver disease. First stage is fatty liver followed by hepatitis followed by cirrhosis and its complications. Cirrhosis is irreversibe and rest all are reversible.
So most important step is abstinence from alcohol to reverse this.
