What Causes Enlarged Lymphnode With Frequent Fever?
Question: FNA OF THE LEFT CERVICAL LYMPH NODE YIELDED GRANULAR PARTICULATE MATERIAL. SMEARS ARE CELLULAR. THEY SHOW DIFFUSE SHEETS OF LYMPHOID CELL CONSISTING OF AN ADMIXTURE OF MEDIUM AND LARGE SIZED LYMPHOCYTES ALONG WITH LYPHO-GRANDULAR BODIES, ENDOTHELIAL CELLS AND PLASMA CELLS. THE LARGE LYMPHOID CELLS SHOW CLUMPED CHROMATIN WITH OCCASIONAL NUCLEOLI. RARE BINUCLEATED CELLS SEEN BUT NO CLASSICAL XXXXXXX STERNBERG CELLS ARE SEEN. NO EPITHELIOID CELL GRANULOMAS SEEN. THE PATIENT IS AN EIGHT YEAR OLD CHILD WITH A ENLARGED LYMPH ON THE LEFT NECK FOR LAST THREE TO FOUR YEARS. PREVIOUSLY THERE WAS NO PAIN BUT SINCE LAST MONTH THERE IS SLIGHT PAIN ON THE OPPOSITE SIDE OF THE NECK. SHE HAS FEVER FREQUENTLY ALSO. THUS AS THE LYMPH GOT ENLARGED FROM PREVIOUS POSITION AN FNAC WAS PERFORMED WITH THE RESULTS ABOVE. WE WANT TO KNOW WHAT IS THE EXACT DISEASE AND ITS TREATMENT.

With an enlarged lymphnode for long time and frequent fever, possibility of lymphoma, particularly Hodgkin lymphoma is there. FNAC is not ideal for diagnosing lymphoma. Also the report provided is suspicious for lymphoma. Hence I think she needs a biopsy of the lymph node where an entire node needs to be removed and examined.
Even if it is lymphoma, excellent treatment options are available.
