i suggest you to take 2 TSP of aristozyme half an hour before each meal three times a day. i also suggest to take tablet PAN D once a day before breakfast.
Avoid onion and root vegetables. refrain from coffee, chocolate n peppermint.
eat fresh food and stop process food.

I checked the reports.
Yes the GFR is declining so are the creatinine and Urea increasing.
THe Alkaline phosphatase is also high which means the kidney function is compromising too.
You should act quickly and speak to your Diabetologist or consult a Nephrologist.
I could also see the blood sugar is not under control with raised fasting and high HbA1c levels.
Please see to your doctor NOW.
