I am Dr.Ajay Panwar, a neurologist, here to answer your query.
I have read your previous discussions with fellow neurologists and read your clinical history.
Your present float offs(likely complex partial seizures) on the background of seizures every 4 to 5 weeks(probably generalised seizures,petit mal is also a generalised seizure type), definitely indicate that you must undergo a brain imaging(like MRI).
Possibilities on MRI-
1)may be normal-MRI comes out to be normal in many cases of generalised seizures(Generalised seizures can be idiopathic sometimes and sometimes with drugs or alcohol)
2)Space occupying lesion(SOL)- like a infectious granuloma or a tumor may be present sometimes,causing partial seizures which may secondarily generalise.Float offs may occur with SOL in temporal lobe of the brain.
3)Sometimes strokes may present with seizures(unlikely in your case)
4)Metabolic disturbances like uremia,abnormal blood sugar or abnormal serum electrolytes may reflect some changes in MRI causing seizures sometimes(unlikely in your case).
Your MRI may turn out to be either of the first two possibilities mentioned above.
