Brief Answer:
You may need antibiotics.

Detailed Answer:
Hi,
Thank you for using Healthcare Magic. The foul smell is a result of the build up of secretions and discharge that would have normally been removed when you replace you tampon. At the moment, you do not need to be worried. I suggest you have a thorough wash and cleanse. You need to be aware of signs of infection developing. These would be a general feeling of being unwell. Also, fever and a continuing of a foul smelling discharge, lower abdominal pain such as cramping. If any of these symptoms develop then see your doctor as you would need antibiotics.
I hope this is of help. If you have any further questions, please do not hesitate to contact me.
Regards,
Dr K A Pottinger,
