After reading your query with diligence it appears that this is a recurrent type of infection known as "Intertrigo" which is occurring again and again. Even then I suggest you to kindly upload a good resolution picture to confirm the condition.

Intertrigo are common type of infections which can affect anybody at any site. But more commonly it affects the areas which are more prone for sweating or moist areas (as in your case). More specifically these are related to the hygiene and environmental factors along with certain systemic complaints as in hormonal problem such as diabetes.
"Intertrigo" is basically infection and inflammation of the crural folds and area below tail bone region which is caused by multiple agents. Though it can be caused by bacteria or fungus but in more commonly by fungus.
Once occured, it has a tendency to recur again and again due to moistness of skin as well itching, pain and scratching the area.
I am providing you certain suggestions for the betterment which are applicable for the condition:
- Keep your self away from hot and humid climate.
- Take two times bath.
- Wear cotton clothes and cotton undergarments to be changed two times a day. Do not wear any synthetic or tight garments.
- Do cold compression or warm water compresses according to the weather condition.
- Do not sit continuously for long hours. Do all the measures to prevent sweating at this area.
-If I were your treating
dermatologist then would have suggested to apply a mixed cream containing fluticaosone with anti-fungal creams such as "Sertaconazole" or "
clotrimazole " for 2 weeks and then shifted to plane anti-fungal cream to maintain the good effect for next 2 weeks.
-I would have preferred to give an oral anti
histamine like
loratadine two times a day if there is moderate to
severe itching is present.
-Don't try to scratch or rub the area as can lead to further aggravation of the problem.
- You can use any good antiperspirant powders daily to avoid future infection.
- Reduce your weight if it is on higher side.
You can follow the above regimen for 10-14 days. If the rash still persists then can go for blood investigation to check for your sugar status.
You need to prevent all the above conditions of moistness or humidity to prevent fungal and
bacterial growth.
