Brief Answer:
Not likely to be a screw.

Detailed Answer:
Hello,
Thanks for posting your query.
The hard small lump near the incision line can be due to an overgrown scar tissue or a small fibroma. The loose screw is also associated with painful movements and it takes many days of pain after which the screw gets loose.
To confirm this you can get an X-ray of your spine done which will show the placement of the implants. Do consult your orthopedician for a direct examination.
I hope this answers your query.
In case you have additional questions or doubts, you can forward them to me, and I shall be glad to help you out.
Wishing you good health.
