What Causes Headache And Swollen Cheeks With Ear And Jaw Pain?

Question: My companion suddenly has swollen cheeks, pain alongside her ear, headache, pain in her jaw, and can't open her jaw very wide and it is painful. Any thoughts?

Based on the history you have shared, it is least likely to be a problem with arteries or nerves as painful swelling and tenderness rules out any si h origin and favors the infection part more. If it had been nerve involvement like we see in cases of trigeminal neuralgia, swelling may not have been the finding though on the other hand any swelling in this area dou to local inflammation may later on compress upon nerves in this area like trigeminal nerves or facial nerves to give such symptoms. Most likely structure to be affected in this area is parotid gland which is located on cheek bone. It can be inflamed by viral infection like mumps, or bacteria like staphylococci. Rarely, sialolithiasis aka stones in salivary gland, TB or autoimmune causes are there, but such acute inset of swelling makes them less likely.

Treatment aims at, staph. infections with antibiotics like augmentin or cephalosporins and anti inflammatory drugs like acetaminophen or ibuprofen. Viral parotitis resolves itself within ten days.

Is TMJ a possibility?

Yes it is, but after parotitis

Hi! Yes it is a possibility, but you have to rule our the parotitis first, and TMJ is in second most possible differential in her case. I did'nt mention it to over flood you with loads of information. Also the pattern of swelling favored parotitis more.

TMJ inflamation is usually localised, though inability to open it completly, does favor that. In any case, staph cover might be needed prohylavtically and Xray TMJ and ultrasound will Rule out TMJ.
