What Causes Heart Palpitations, Dizziness And Metallic Taste In Mouth?

Question: three days ago i got heart palpitations, dizziness, shortness of breath and a metallic taste in my mount it to a point where my wife had to drive me and i went to the hospital. I explained the symptoms, they put me on O2 monitored my vitals and took blood. No indication that there was anything wrong - blood work came back negative. Back in 2004 i was admitted for chest pain where they stress tested, ultra sound of my heart and walked with a portable heart monitor - negative. 2012 i suffered what my doctor thinks is vertigo on the golf course and nearly colapsed - went in to the hospital for stress testing and other tests - negative. No diabetes, no indication of anything wrong.
Blood pressure is normal - heart rate a rest 53-58 hps.
Married - 37 years - faithful.
I smoke cigars daily and inhale only a little - too strong otherwise. I drink a 2 - to 3 beer per day. Usually 1 large coffee per day - usually eat little in the morning - light lunch and dinner. Hard food - vegetables and meat - very little junk food.
I used to work out years ago rigorously - karate, weight lifting, sailing. I am do applied mechanical engineering and work as a physicist working in the field of stellar and plasma physics - differential diagnostic techniques and head up a team of Doctors in this field. I work approximately 180+ hours up to 230 hrs per month in office and lab.
Right now my life is sedentary but do play golf / driving range when i can - seasonal - Canada.
I have sensitive ears and have had a history of inner ear infection and got one this past winter - took decongestant to allow drainage due to Eustachian tube congestion - normally works and can clear the ear - normal wax build up - but on occasion still feel like there is slight pressure against the ear drum - most notable when cold air gets in.
I do not feel pain in my chest but more of a empty feeling and general weakness and dizziness just after i got home sitting at my computer.
