Question: For sometime now, I have been having heartburn, bloating stomach, discomfort bending over, discomfort lying in bed on RHS, tooth decay (when I have always previously had excellent teeth, with only the odd filling). Condition considerably worsening. Severe dental problems, blocked nasal passages, and mucous in my throat, and now at the back LHS of my tongue. I get hiccups, usually followed by vomiting - nearly every day.

You are suffering from gastroesophageal reflux disease (GERD), which is due to lax sphincter between esophagus (food conduit) and stomach and hyperacidity causing reflux of acid stomach contents into the esophagus. This causes heartburn and bloating in stomach, hiccups and sometimes vomiting. Certain patients suffer symptoms unrelated to esophagus like tooth decay (Caused by acid related damage to the teeth), mucus in the throat and back of tongue (Increased salivation to neutralize the acid refluxate), blocked nasal passages.
If I were your treating doctor, I would do upper endoscopy to assess the damage caused by the acid to esophagus an stomach, start you on omeprazole 40 mg twice daily (before meals).
I would also recommend you to lose any extra pounds of weight and be out of stressful life whatsoever

Thank you for such an excellent explanation. My other queries, if I may please ask:

1) In your opinion, could this (also) be a Hiatus Hernia?
2) Cause of GERD - is it Cancer related?
3) I am not overweight - if anything, underweight. I have tried as much as I can from online information, changed my diet quite considerably. Become vegetarian, eating 'easy to swallow' foods, non spicy etc. Usually only eat a small amount, once a day.

The only way to see a hiatus hernia is to directly see it on an endoscopy
Cancers in the esophagus usually cause blocking sensation on swelling food or fluids with significant weight loss and i don't seriously suspect it in your case as you don't have one.. So don't worry
Having said that, a pre cancerous problem like XXXXXXX s esophagus is seen in people with GERD. For you to understand, the skin inside your esophagus changes it's nature to tolerate chronic acid exposure and is called "barretts "
So to look for all these possibilities, your doc has scheduled you an endoscopy next week
The recent guidelines on GERD issued by XXXXXXX College of Gastroenterology recommend against any specific food restriction and wipes out all the traditional beliefs
Infact your food restriction may be causing you underweight

Thank you. And lastly:

1. I am very active physically, and also have a very 'stressful' workload, which unfortunately I cannot retire from; being single, I have to manage my own finances.
2. Re Endoscopy: Can they detect immediately/give results immediately.
2. I am on Omeprazole, although was only taking 20mg per day. I will increase dosage x 40mg twice a day, after endoscopy. I'm advised that prior to endoscopy I have to stop taking any medication for at least a week beforehand.
3. What is the prognosis - is it life threatening, does it recur and can it be (successfully) treated without hospitalisation.

The results of endoscopy are almost always provided immediately before you go home
GERD is treated in outpatient clinic most of the times and is not life threatening
But the associated discomfort and misery may impair quality of life
Stress is very important factor that needs to be addressed for long term cure of this disease. You can try yoga therapy for relaxation
Finally I don't find any logical reason to stop medicines before endoscopy
