This is not due to Diabetes or psoriatic arthritis.

Detailed Answer:
Hi,
Thanks for being on healthcaremagic.com.
I am Dr.Ajay Panwar,a neurologist,here to answer your query.

The hissing sound you are mentioning is most likely 'tinnitus'.It is unlikely to be due to Diabetes or Psoriatic arthritis.

Please tell some more details about it so that I can answer this well-
1)Since when are you having this hissing sound?
2)Is it persistent all the time or improves intermittently?
3)Any ear related complaints-hearing loss or ear discharge?
4)Any history of headache, fits or loss of consciousness?

I shall be glad to have you in follow-up with details.

Brief Answer:
Tension type headache/ENT evaluation

Detailed Answer:
Hi,
Thanks for being in follow-up and providing further details.

In view of history of constant dull pain at the base of the head, it appears to be a 'Tension type headache'. Hissing sound as you mentioned can be a part of Tension type headache.

For now, please start doing regular mental relaxation exercises like meditation/yoga and make your life as stress free as possible. If the symptom still persists, please get a CECT scan of head done to rule out any intracranial pathology and at the same time, please get evaluated by an ENT specialist for tinnitus.

Hope that I have answered your query. If you have some further questions,I shall be glad to have you in follow-up else please close the thread, rate it and write a review.

Regards
Dr.Ajay Panwar,
