Your symptoms seem to be related to possible stroke. The fact that your symptoms are more prominent while standing up and are not changed by head movements during sitting or lying down position, is an argument in favor of possible stroke in the posterior brain (also called posterior circulation). Otherwise, it would indicate an inner ear disorder, but this cause is less probable in your clinical situation.
I would recommend going to the ER for a careful physical exam and a brain CT scan or MRI to exclude this possibility.
Further tests need to be done:
- a Doppler ultrasound of the cervical arteries to examine the blood flow to the brain
- resting ECG and cardiac ultrasound to examine your cardiac function
- blood lab tests (complete blood count, blood fasting glucose, blood lipid profile, kidney and liver function tests, etc.).
