What Causes Indigestion In An Elderly Person After Taking Metronidazole, Omeprazole And Clarithromycin?

Question: THIS QUESTION IS DIRECTED TO A DOCTOR OF XXXXXXX ORIGIN

Patient is 65 years old female of XXXXXXX origin ,currently residing in USA. Patient showed adverse reacton to h-pylori bacteria treatment which was treated in USA with prev-pac---with combination of 3 drugs such as prilosec,clarithymicine & flagyl. The high dosage of these combination has made her digective symtoms worst and she has to stop it after 8 days. she developed symtoms of constant burping, indigestion, loss of apetite . One gasentrologist suggested that misbalance of bacterial duct may have happened and cud be small intestine bacterial growth. I have also contacted XXXXXXX gasentrologist. They have suggested that patient may have developed dyspeptic & reflux symtoms and to get 2 medications from India-----
1. lldac-L 1-0-0 & 2. VSL #3 1-0-0
Her gasentrologist in USA has tried probiotics but its not working.
My question is, should i get those two medications from XXXXXXX to treat her symtoms? Do you suggest to get any additional medication from India?
Patient is also taking blood presuure medications
Please suggest.

See, VSL 3 is a Probiotic and there are not much of any major difference between Probiotics ( all are the same ). So there is no need for VSL 3 if a course of probiotic has been given already.

In my opinion, Dyspepsia is the right Diagnosis. It might have happened because of irritation to the gut , other possibility could be loss of Digestive enzymes of the gut due to Antibiotic used. It mainly cause Indigestion as a side effect.

I recommend to you Digestive Medication like Syrup Aristozyme (enzymes) for 2 weeks and a prokinetic drug like Levosulpride for 5 days. Avoid eating very oily food, non veg, eat more of fruits and Vegetables (except Potato ), avoid cheese and butter.

Also I need to know whether the H.Pylori treatment was started after any tests ( Endoscopy ) or it was started based on patient's symptoms.

Respected Doctor,
thanks much for your reply. Sir,basically the patient always suffered gas problems and at that time she was treated in XXXXXXX and for some reason the XXXXXXX gas medications worked on her. This was like 15 years ago. For last 10 years she is in USA. Now recently like 1 year ago she complained about gas problems so we took her to gasentrologist in New York
They did endoscopy and stool exam as a normal procedure and everthing came ok except for H-pylori bacteria. Doctor suggested there is no need to treat h-pylori unless there is symtoms . But her regular doctor suggested to treat it as it may turn into ulcer. So they used a medication called prev-pac which is combination of 3 drugs namely Prilosec, Clarithomycin, Flagil. it seems like the dosage was too high for her and she has to stop after 8 days and was taken to ER. This was end of September 2015. Still today she cannot totally come back to normal life with those side effects. And we feel that her gasentrologist is not treating the real problem except suggesting probiotic medications available in any store .
Her symtoms for last 4 months shows---after every 2 hours she is getting hungry, feeling weak, feeling not to eat, feels like as if food is stuck in throat. After eating she is somewhat normal. But the fear of this attack every 2 hours is making her depressed.However, her recent blood work of liver and all are normal.
Now, we feel that we should treat her with XXXXXXX drugs. So please advice, which drugs should we get?
1. Syrup Aristozyme (as you suggested,for how many days if symtoms persist beyond 2 weeks and how many times a day?). Can she continue to take it until complete cure?
2. Levosulpride (as you suggested , if symtoms persist after 5 days)Can she continue to take it?
3. Should we still get the Iladac?
Any other drugs as a back up?
Can she take the above medications along with her BP medications prescribed in USA?
Please help Sir.

Thanks for the information.

In my opinion there was no need to treat for H.Pylori.

Follow these recommendations :

1. Avoid Dairy products : Milk, Curds , Butter , Cheese and Ghee.
2. Avoid eating very oily food, non veg, eat more of fruits and Vegetables (except Potato ).
3. Aristozyme can be continued for 2-3 months also , to be taken with food 3 times a day. But do discuss about this with your treating doctor.
4.Levosulpride can be extended upto 2 weeks but not more than that, but it can be taken as and when required later.

Medications can be taken with her BP medicines.

Let her follow the diet , take these medicines , go for brisk walking everyday, if possible learn Yoga and do it everyday. If inspite of doing all these she doesn't notice any significant change then I recommend her to get Colonoscopy with biopsy of intestine , tests for Celiac Disease and Serum Lipase tests.

Dear Doctor,
thank you so much for your help. I have one last question Sir.
Please let me know if GR8 capsules and Librax tablets from XXXXXXX will help in this situation. these were recommended by an XXXXXXX doctor who one my relatives contacted in XXXXXXX
So i will bring all medications together and get these evaluated by our USA doctor and make a final decision which one to use.

No need of GR8.

Gr8 is pretty much similar to Levosulpride but a milder tablet.No need of it if you are getting Levosulpride.

Librax is a sedative/anti-anxiety drug. It can used to calm her down or to make her sleep well but it's of no use for her indigestion problem.
