Question: What do I do?
I'm on medi-cal and it sucks. I suspect I have some gall bladder disease as I seem to have major ibs and gastric dumping syndrome symptoms, especially after heavy meals, most meals that involve meat, and if I don't eat anything for a bit and then eating anything symptoms happen. Under Medi-cal my doctor literally said 'I could not see a specialist, until I started throwing bile.' It has now escalated to shoulder pain...im at a loss...I can't afford decent healthcare, and no medi-cal isn't 'decent.'

I have gone carefully through your query and understand your concerns. I would like to help you but I would need you to elaborate on the complaints. Please mention how and when it all started, what triggers/precipitating factors or relieving factors were there? Please mention if there was nausea and vomiting whenever you took food and whether fatty foods made it worse. Also mention if an ultrasound of upper abdomen (or whole abdomen) has been done. If so, please upload the reports, if you have them at your disposal. It is very important to get an ultrasound of the upper abdomen done.

I do not suppose that the shoulder pain is related. However, I would need you to provide me details for it as well. It could be due to a muscle spasm. The vicious cycle of pain and spasm needs to be broken and I would insist you to take paracetamol or acetaminophen 500 mg two tablets together twice or thrice daily for next 3 days. This should ease the problem and make it go away. If it does not, I would insist you to get a X Ray done.

I started noticing sensitivity a little over a year ago...when the first time I literally shit my pants right after a fatty (meat) included meal. Thank god I was alone in my car and could run home without anyone noticing. Nausea happens, insane bloating (sometimes even enough to where a have to loosen the belt one notch), after meats, fatty meals. Insane diarrhea right after meals sometimes literally within minutes. Alcohol consumption, along with sugar drinks, seem to assist the problem. Some relief has come from adhering to a mostly vegetarian / healthy lifestyle. No ultrasound was done anywhere. I feel uncomfortableness in my right abdomen when i sit and lay down (not standing) and have to sleep on my to relieve it. It feels like something is swollen and/or something is pushing against my rib cage. Applying pressure and massaging relieves some. no vomiting yet.

Thanks for writing back. Your problem seems to be related to your irritable bowel syndrome and could be associated with irregularities in your bowel movements. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets affected you get symptoms like the ones you described. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation.

In order to relieve yourself, you need to do the following:
* Avoid spicy and fatty foods. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake.
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as different types of fruits. You need to maximize the intake of soluble fibers but you need to limit the intake of insoluble fibers.
* Regular morning walk and exercise to keep the bowel movements proper.
* Anticholinergic and antispasmodic medications such as hyoscyamine (Levsin) and dicyclomine (Bentyl), can help relieve painful bowel spasms like the ones you are describing. Laxatives (not purgatives) on the other hand would help in reducing constipation while antimotility medicines can help relieving diarrhoea or the sudden urges like the one you have described. Since these are prescription medicines you might need to visit a doctor to get them written down.
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
* Stress reduction has a very important role to play. Yoga and meditation should help.
* I would insist on getting an ultrasound of whole abdomen done. You can upload the reports thereafter.
