What Causes Irritation And Vomiting Post An OPV Vaccination In An Infant?
Question: Dear Doctor,
Today we went for the second vaccination (10 weeks) for our daughter. Couple of things in this regard
1) We could not give her IPV since it was not available anywhere but we had to give her OPV. Hope this is fine, kindly confirm.
2) Our daughter (whom we vaccinated) is feeling a bit irritated today and we also see some redness in the area where injection was applied. Please revert if any specific care need to be taken in this regard
3) Off late we have seen that she frequently (around 5 times in a day) vomits with milk coming out of her mouth in good quantities. We try to burrp her but still this happens. What are your views on this and do you have a suggestion on this?

I understand your concerns but don't worry. You may continue with the routine immunization and give her an extra dose of IPV at 18 weeks of age (for missed IPV at 10 weeks) . This will give her the same protection.
The vaccinations at 6, 8 and 10 weeks are many a times associated with fever, pain and redness at injection site and irritability. You need to apply cold compresses at site of injection along with acetaminophen or Paracetamol (10-15 mg/kg/dose at 6-8 hourly interval) for relief from these symptoms. Ensure regular feeds.
The common reasons for recurrent pukes in this age group are gastro oesophageal refluxes, overfeeding and poor burping. You may ensure on-demand feeding (exclusive breastfeeding is in best interest of baby) and avoid overfeeding. Refluxes usually respond by burping and keeping head end slightly higher while lying. Most babies improve by 4-5 months of age with no requirement of any investigations and medications.

1) When you say extra dose of IPV at 18 weeks, do you mean two such injections instead of one? Moreover this injection (IPV) is not available anywhere in XXXXXXX so can we assume the oral dose is replacement of the same?
2) We are currently giving her babygesic drops. Hope that is fine?

IPV non-availability is usually a momentary thing and hopefully it will be available after a few days. You need to give IPV at 14 weeks as per schedule along with an extra dose at 18 weeks. IPV has added protection with OPV and is not an alternative to OPV. You need to give both.
You may continue with Babygesic. Its fine.

In rare case if the redness increase or fever doesn't improve even after 24 hours from now, then the reason can be post injection abscess. Start antibiotic such as Syp. Augmentin Duo (2.5 ml twice daily) in such a case.
