Brief Answer:
You need clinical examination.

Detailed Answer:
Hi,

Thanks for your question.
Noted your concern.

What do you really want to say by word smaller? in length or thickness?
Change in length is unlikely until and unless it may be present since childhood or there may be bone fracture or bone infection.

Change in thickness means one limb is swollen as compared to another.
There may be various causes for it in diabetics-
- Muscular dystrophy (muscle wasting) due to diabetic motor neuroapthy.
- Cellulitis (ifection of skin and underlying tissues) but in usually associate with pain, redness, fever.
- Oedema due to water retention as in heart, kidney or liver disease (but rarely unilateral).
- Deep vein thrombosis.

Better to consult your treating doctor for detailed clinical examination, depending upon it there may be need for further evaluation.

Hope this helps you.
