Question: My son, 28 yrs (has cerebral palsy since birth), has developed high fever, stomach bloated (stiff / hard) & has severe loose motion (greenish) since last 8-10 days. Just admitted to KEM Hospital -Pune in ICU/DHU yesterday. During ultra-sonography, no signs of his intestine (small & large) due to excessive gas formation. All other reports stool, blood, urine are normal. Any similar cases, please help.

Loose motion associated with distension of abdomen and fever following possibility needs to be considered
1. Toxic megacolon: Usually the individual present with loose motion followed by distension of abdomen and then fever. Diagnosis is based on typical history, clinical examination and erect X ray abdomen.
2. Pseudo-membranous Enterocolitis: Superinfection with Clostrium difficile will usually cause it. The stools are usually blood stained or may show blood on microscopic examination.
I see several such patients presenting to me, I would like to now the following investigation of your son
1. Complete blood count
2. Erect X-ray abdomen: It will be very useful if you could provide the clinical photograph
3. Stool examination.
4. Other investigations done till now
Kindly revert back to me with above details and also complete sequence of events which lead to present admission. Please upload the reports of ultrasound too.
