Brief Answer:
Surgery is a viable option.

Detailed Answer:
My dear,
Thanks for sending your query to us.
If there is field loss ,that means glaucoma is not well controlled by medicines and further steps are required. We have to know before commenting, your vision and intra ocular pressure readings.
Step one is to add another drug or to go ahead with what is called as a filtering surgery, which is always a good option .
In glaucoma where control is not proper surgery is a good option and should be considered.

Brief Answer:
What is the reason of less than 20/20 vision

Detailed Answer:
My dear,
Is there any reason detected for less than 20/20vision like cataract or any other defect . Otherwise this diminishing of vision is attributed to glaucoma and management of glaucoma changes.
You ideally have to be kept under observation and fields repeated after 6 months and then decision to be taken if drops are to continue or go ahead with surgery.

Brief Answer:
Filtering surgery is done in open angle glaucoma.

Detailed Answer:
My dear,

Filtering surgery is done in glaucoma where the angle is open. Here, since there is resistance in the original drainage channels of the eye an alternative pathway is created where fluid from the eye is drained out in the layers of white of the eye. You can google these details under 'Trabeculectomy', Which is the technical term for this specific surgery.

Kind regards

Brief Answer:
Glaucoma affecting the vision.

Detailed Answer:
My dear,

If there is no other cause of you not having 20/20vision, then this will be attributed to glaucoma, and your records should show that before onset of gl glaucoma your vision was 20/20.

This vision is central vision loss, perimeter shows peripheral vision loss. Your treatment with 3 drug region is usually maximum. That leaves us with the last option of 'Trabeculectomy' if fields are not stabilized.
