Loss of appetite, difficulty trying to sleep, shaking, diarrhea on eating and jitters in waking time indicates likely a flare of anxiety with history in past. However the organic symptom of tremors and diarrhea can not be ignored and needs to be evaluated for the cause. I would suggest hormonal workup for thyroid imbalance as hyperthyroidism can present this way. After that, i would suggest workup to rule out irritable bowel syndrome or Inflammatory bowel disorders.

Paxil is a paroxetine and a very safe SSRI so yes you may take it for a month of two. If you feel like you need another opinion you can have it, but psychotherapy, rehab sessions help with such psychological flares more than these meds. Learning how to cope up is needed.
