What Causes Low Grade Fever And Loss Of Appetite?

Question: I am 59 years female . I am having low grade fever (Usually between 98.6-99.1) in the afternoon and late evening for the last 3 weeks (daily without a single day off). During daytime I am normal. My symptoms are
1) Afternoon & Evening rise low grade fever.
2) pain in neck (X-ray and MRI done and finding are osteophytes noted in C5-C6)
4) loss of appetite
5) lathargic
No other symptoms like burning during urination etc
I have undergone through quite a large number of diagnosis tests
1) ANF/ANA -ve
2) Malarial Parasites -ve
3) Dangue -ve , LgM Ab to S.typhi -ve
4) Widal
5) ESR is high == 91 MM/1ST HOUR
6) CRP = .96 MG/DL
7) HBA1c,CALCIUM,BLOOD HEMOGRAM (all within range)
8: TB gold (report awaited)
I don't have sugar,B.P.,uric acid
kindly suggest why can be the reason of fever

I,m Dr Sudhir XXXXXXX

Looking to his symptoms and report I consider first possibility is tuberculosis of chest or some abdominal organs.

Following symptoms and reports suggest tuberculosis.
1. Afternoon & Evening rise low grade fever.
2. loss of appetite
3. ESR is high == 91 MM/1ST HOUR

She has pain in the neck but MRI does not favor neck as involved region.

TB gold report is already awaited.

I will like to do high resolution computerized tomography (HRCT )scan of abdomen and chest to see abdominal and chest tuberculosis. This will also exclude some malignancies presenting similarly.

Another test I will like to be done is bone marrow biopsy to rule out leukemia however blood reports does not point towards it and further it is a rare cause.

Her TB GOLD report is positive. It means that she has Tuberculosis infection(latent or active) but not necessarily active tuberculosis disease. Her chances of having tubeculosis as cause of fever has increased.So I suggest previosly advised tests.In addition I will suggest lymph node biopsy of neck if any of lymph node is enlarged. Empirical antitubercular treatment can be started after consulting tuberculosis specialist(TB and Chest specialist).Improvement in symptoms will be there in nearly 1 month if she has tuberculosis.Definately it is cause of concerned till final diagnosis. Presuming it tubeculosis its treatment will take nearly 6-9 months.

Just got Mantoux test result it says 08 mm and urine culture showed no growth.

pls suggest,shud medication for TB be started,what are the side affect of medicine and dos and don't for TB infected person as in what to eat must and not eat etc.

Welcome back.

Your chances of having tuberculosis as cause of fever have increased. So I suggest previously advised tests (high resolution computerized tomography (HRCT) scan of abdomen and chest and lymph node biopsy of neck (if any of lymph node is enlarged) to search for site of tuberculosis and to exclude other causes.

Empirical anti tubercular treatment should be started after consulting your treating physician or tuberculosis specialist (TB and Chest specialist). Improvement in symptoms will be there in nearly 1 month if you have tuberculosis. If no improvement is there, it may not be tuberculosis.

Side effects of these medicines are liver toxicity, temporary vision disturbances, nausea and many more so at regular intervals (15 days to 1 month) liver function tests, examination of eyes and blood investigation are done to see these side effects. YOU SHOULD TAKE HIGH PROTEIN DEIT (egg, pulses,milk and milk products) and nothing is restricted in diet.
