If you would have presented to me as an outpatient with this complaint,I would have made a probable diagnosis of Lumbosacral radiculopathy(mainly,lumbar nerve roots involvement) on the basis of medical history.
I would have suggested you to undergo the following investigations in order to establish the diagnosis-
1)MRI Lumbosacral(L-S) spine
2)Nerve conduction studies of lower limbs.
Please get examined by a neurologist and discuss the possibility of getting these investigations done.
Once you are done with these investigations,please follow-up with the results here.
Further,I would also like to rule out Diabetes(as Diabetes can affect nerves resulting in varied manifestations) in your case.So,please get blood sugar (fasting) done and share the result here.

Meanwhile,you can start therapy with anti-neuralgic drugs like pregabalin,amitriptyline or duloxetine.Besides,methylcobalamin supplementation will be helpful.

We can modify the therapy,once we get the results of the investigations.
