What Causes Numbness On Feet And Coccygial Pain?

Question: Hello Dr. Grief, how are you? My name is XXXX. I am a 35 year old white male, 5'8", 200lbs. In october of 2012 I went to a wedding in Connecticut, and wore a new pair of dress shoes all day. The next day I was in New York, walking all day in a pair of New Balance 992, which were brand new. About mid-morning the balls of my feet were hurting bad, as if the bone were directly contacting the concrete. I walked like this all day, and the pain finally subsided a few days after.

Fast-forward to February 2013. I did not wear those dress shoes again until a funeral, and had the same pain happen. During the following summer (about a year ago) I noticed two things happen simultaneously.
1 - my sacrum/coccyx hurts as i get up out of a chair, and sometimes hurts while sitting longer than 10 minutes
2- my big toe on each foot is fairly numb, and there is a loss of sensation over the other four toes. I started walking this week and the numbness in the right foot becomes really noticeable, while the left foot stays relatively manageable.

The numbness and coccygial pain were almost simultaneous. There was no tingling, burning or any progression with the numbness in my feet. It was a sudden onset. My feet heal fine, are warm, and when I do get the occasional scrape (my feet have always been dry and sometimes a minor "break" happens on my foot) my foot is healed in about a day.

And, my feet stay the same numbness all day, from when I get up to when i go to bed at night. There is no pain. I can feel a little bit when i brush my toes over carpet, but the sensation is lessened.

So perhaps its type 2, or b12 deficiency, or sciatic nerve related. No clue here. Help!

You have given a detailed clinical history. I would recommend you the following:

- MRI spine to rule out a vertebral disc disease and sacro ilieitis
- Vitamin b 12 levels in the blood
- Fasting blood sugars.

I would recommend you to have a clinical examination done by an orthopaedic doctor. SLR test is a clinical test to diagnose sciatica and vertebral disc disease. Peripheral sensory examination will help us to localise the problem.
