Hello again.
Rhythm abnormalities should be ruled out during such episodes. An EKG can be done. Serial EKGs can be performed or 24 hour holter monitor can be advised if there is any doubt. 130/70 is a normal BP reading. With 116 /46, the diastole is low. In Diastolic hypotension cardiomyopathy should be looked for. Echocardiogram, blood counts and serum electrolytes including calcium would be appropriate to advise. Low DBP with a widened pulse pressure indicates high arterial stiffness and atherosclerosis. It can increase the chances cardiovascular event. Lipid profile can be advised and initiating statins like atorvastatin can be considered, in consult with a Cardiologist.
