What Causes Oral Sores Post Oral Sex?

Question: Hi Dr.

I tried messaging you on our open forum but it was saying I could not ask on that question anymore. But I need your advice and you already know my situation.

I have some possibly bad news. I had oral sex with my partner a few days after my encounter. A few days after she complained of having some oral sores which we thought were canker sores at the time. They never developed into canker sores and they are now gone. Could this be a possible STI? If so which one?

I have not received my test back for Chlamydia and Gonorrhea. How long does it normally take to get a result? Is there any other STI I should be worried about and test for?

I am not having any sores or symptoms of STI's on or in my genitals. Is it still possible I could have passed something to her?

I am very anxious now after hearing this.

Please Advise.

Detailed Answer:
Hi. Welcome back.

STD's can present with Oral ulcers/ sores.

STD's commonly presenting with Oral sores include Herpes (herpetic gingivostomatitis), syphilitic chancre and rarely chancroid.

Syphilitic chancre is painless. Chancroid is very painful and friable and unlikely to heal on its own.

This leaves us with either Herpes Or Canker sores.
Herpes gingivostomatitis too is very symptomatic with multiple painful sores involving gingival mucosa, hard palate. Patient with herpetic gingivostomatitis usually is in a lot of discomfort with excessive salivation and drooling..
Therefore, most likely your partner had aphthous stomatitis Or canker sores.

Differentiation between herpes and canker sores is also clinically possible.
Herpetic sores usually involve mucosa overlying bone i.e adherent mucosa e.g gingival mucosa, hard palate etc whereas canker sores/ ahpthous ulcers commonly involves freely movable mucosa/ soft mucosa that does not overly bones e.g undersurface and lateral surface tongue, floor of mouth, buccal mucosa, lips and soft palate.

Urine test for Gonorrhea and chlamydia should be available in 24 - 48 hours. It is usually available on same day.

Hi Dr.

Thanks for your prompt response. My partner does have Herpes 1 and sometimes get blisters on her lip. This were small circular sores, looked like canker sores but did not develop all the way to painful canker sores.

In your opinion you believe this is either the herpes 1 or the canker sore, not chlamydia or gonnorhea? Can you also explain syphilitic sore?

May have been an abortive episode of canker sores Or EM Minor

Detailed Answer:
Hi.

Canker sores may not develop into full blown sores and sometimes may just present as a self abortive episode.
Recurrent herpes can also present with a few oral sores, here and there, considering that she already has herpes type 1.
Recurrent herpes is milder unlike primary herpetic gingivostomatitis which i mentioned earlier. Primary herpetic gingivostomatitis is very severe and symptomatic.
Another possibility is that she is had an episode of Erythema Multifome Minor which is commonly associated with herpes labialis i.e herpes associated erythema multiforme Or HAEM.
EM minor can also present with sores or red patches in Oral mucosa and on labial mucosa.

These sores which she developed this time round, were they inside her mouth Or were they on lips? Can you tell the exact location?

Gonorrhea and chlamydia affects pharynx/ throat rather than oral mucosa and present as sore throat.
On examination, pharyngeal mucosa would be red and inflammed.
Gonorrhea and chlamydia anyways do not present with oral sores, so this is unlikely to be the cause.

Syphilitic sore is painless, well defined, clean based sore. It has a hard/ indurated base which is palpable. There are also enlarged regional lymph nodes.

To summarize, these sores which she developed doesn't seem like an STD to me

These sores are in the inside of the mouth, one was inside the mouth near the cheek and another was across the mouth where the front teeth are (next to the gums).
Quick follow up, the Chlamydia and Gonorrhea results came back negative :)

Are these test conclusive in this window? I don't need to take another of these again will I?

I spoke to the Dr. who reviewed the results and did my test, he also said that the sores in her mouth were probably related to something else (like canker sores). I appreciate your guidance through all of this, you are a live saver!

Negative urine test result for gonorrhea and chlamydia is reliable and conclusive in your case since it was done at an appropriate time i.e past the window period for this test.
