What Causes Pain And Discomfort In Scrotum Post Ejaculation?

Question: Hi dokter, This is pretty embarrasing, but I can't find an answer and this is anonymous, for which I am thankful in advance.
Since 4 weeks I have post ejaculation pain/discomfort, which starts in the scrotum and then also in my groin area both left and right, the area where the spermatic cord is situated. It feels like some kind of lump development and swelling which dissolves after 6 days of rest (no ejaculation)

Now comes an even more embarrassing part, I started jelqing some 10 weeks ago, and not very frequent or excessive (I should have never done that). I did that maybe 2 times a week for ten minutes. After that I measured my penis with a ruler in erection before ejaculation.
The ruler I pushed on the side in the lower groin area, just where the spermatic cord is (I then didn't know the anatomy). The ruler is from hard plastic. *Could this cause trauma and or blockade, inflammation of the spermatic cord or the Vas deferens, or could it influence negative the vacuum process which starts right before actual ejaculation?

I don’t have pain in the perineum, the uretha and the penis and I have been tested negative for STD's. It swells just around my balls right and left and the groin area. The achy feeling seems to change location during the day (lump?)

After a week rest, the symptoms where there again. Now I have initiated a new rest period, currently in day 11. I do feel the spermatic cord, or the area, no pain though. Hoping that also this feeling goes away after a few more days.

*Could this cause trauma and or blockade, inflammation of the spermatic cord or the Vas deferens, or could it influence negative the vacuum process which starts right before actual ejaculation?
**- Or could it be irritated muscles around the pubic bone, lower inguinal opening which are used with ejaculation and which affects the spermatic cord and then the scrotum

Hello XXXX and welcome to HCM.
As an Urologist, i can fully understand your concern.

You need not feel any embarrassment, asking any questions to a doctor.
Jelqing is practised by many persons.
It's effects aren't guaranteed to cause changes in penile length or erections.

Your symptoms are suggestive of inflammation of the spermatic cords, or prostatitis.

What you need to do is :
1. Get examined by an Urologist, so a local check-up of the genitalia and rectal examination(DRE), is done.

2. Get urine routine and culture tested.

3. Blood routine,RBS,creatinine,platelets and LFT done.

4. ultrasound scan of abdomen and TRUS.

5. Scrotal ultrasound Doppler scan.

According to these reports,your problem should be clear and specific treatment can be started.
For an expert opinion,you can send the reports here.

Kindly mention if you've had sex, with multiple partners or had STD earlier.
Avoid sex till reports are ready and examination is done.
