Symptoms you describe seems to be related more to peripheral neuropathy and low vitamin B12 is one of several causes that may cause it.
EMG done 6 years ago isn't relevant in the present, so, in my opinion, a new EMG is necessary at this moment.
If you have psychological issues you should address to them by consulting a Psychologist, but neuropathy related symptoms should be solved first.
If strong evidence of neuropathy after EMG study, treatment about it should be started as well.
Peripheral neuropathy symptoms respond in a satisfactory degree to drugs such Pregabalin (Lyrica), Amytriptiline, Gabapentin (Neurontin).
I suggest you to discuss with your treating Doctor about above mentioned possibilities.
