He says it sometimes hurt when he breathes. And he's always had this problem but recently i noticd he's been massaging the area under his left collar bone more and more.
Brief Answer:
He needs to get an xray done
Detailed Answer:
Hello
There is a condition called as 'cervical rib' in which there is abnormal growth or calcification from the first cervical vertebrae, which is a bone of the back.
The abnormal growth can cause pain in the area which your boyfriend is feeling pain and the best way to visualize this is by an xray. Please remember this is just one of the possibilities as i have not examined him.
Another more common possibility would be a muscle pull/strain, for which the best thing would be to meet an orthopedic doctor. he may advice an arm sling, in order to rest the muscles and avoid lifting heavy weights.
For the time being you can apply heat to the affected site and ask him to avoid straining or lifting weights.
Take care
