I'm Dr. Sudhir Kumar,
After going through your query I concluded that you may be having adhesive capsulitis of both shoulders as you have pain in shoulder and if you squeeze shoulders forward, it feels like bones are tearing off from chest. It is also called frozen shoulder.
I want to know some more information from you.
1.Since when you have this?
2.Is movements of shoulder joint restricted and painful?
3.Whether you are diabetic?
4.What is x- ray finding , you have already done?
Treatment of this is mobilization exercises of shoulder. Analgesic such as ibuprofen is helpful in pain relief and facilitation of exercises of shoulders. Local analgesic sprays will be of benefit to spray before 5 to 10 minutes before exercises.Sometimes steroid injection is given in joint to facilitate exercises. Vitamin B and C is helpful in recovery. You can discuss with your treating Doctor about it.

No restricted movement, just a little on the right shoulder. Physiatrist says not frozen shoulder. :( he does not know what is wrong. Pain only when shoulders pushed forward

After going through your initial query and follow up query, I still consider partially recovered frozen shoulder as first possibility as you also had this couple of years back and did have restricted movements earlier.
Second possibility can be vitamin deficiencies specially vitamin D3 deficiency as you also have generalized chronic pain and muscle weakness at times, better with diet changes. You can get your vitamin D3 and vitamin B12 levels.
Thyroid hormone deficiency can also give such symptoms, it is advisable to get thyroid function test (T3,T4,TSH).
Your neck arthritis and fusion may also contribute to some weakness and muscle weakness and pain.
Treatment remains same as already discussed but multivitamin and trace elements supplements, Mecobalamin and VitaminD3(if it is low) can be added.
