Question: I have a severe headache behind left eye and temple...and noticed recently several long, narrow (less than a centimeter) indentations in this same area...like if I were to scratch my head in area, the indentations catch my fingertips?? I have a history of migraines, but they have been controlled and this pain is worse??

Brief Answer:
Need a medical examination along with a visual acuity

Detailed Answer:
Thank you for submitting your question for consideration. I have read it and would like to make the following recommendations based on the symptoms. If you are having a situation where you believe you feel something under your fingertips along the temple area and you have a severe headache behind the eye on that side then, I believe A thorough medical examination (preferably by a neurologist) would be to your benefit. In addition, a good visual acuity should be performed on your eyes and a good history should be taken by the physician to rule out any type of pathology that could affect your vision on A permanent basis.

I can tell you that based upon your age the likelihood of you having any type of serious pathology that would affect your vision on the basis of the symptoms is very low. Nevertheless, because of the way you describe things I would still maintain that a good neurological examination and visual acuity testing be performed just as soon as possible. if with in the next 24 to 48 hours in fact, you begin to develop blurred vision or dimming in anyway then, the need becomes urgent and you should proceed to an emergency room.

Otherwise, the likelihood is that the pain you are experiencing is simply a recrudescence of your migraine headaches. You state that they are under control, however, I note that you are taking Fioricet and ibuprofen as your medications of choice. As a neurologist and headache specialist the Fioricet is not One of my drugs of choice and in fact is one that I tend to avoid and try to remove from patients who come to me on consultation. What we find in terms of that particular medication is that a tolerance develops to it rather quickly and headaches that breakthrough tend to be more severe and require more treatment at higher doses. Furthermore, that drug and drugs such as Fioricet do nothing to treat the underlying cause of the headache which should be look for by way of either a headache diary or a headache specialist.

If indeed you are having headaches that are definitely migraines by definition then, the drug of choice at this time comes from a class known as the triptan medications. Examples of those medications include Imitrex, Maxalt, and Zomig among others. narcotic medications or semi synthetic narcotic medications such as all tram or tramadol are not good long-term choices in someone with chronic headaches.

I hope these responses and insights have provided you with some additional useful information that you can discuss with your physician and that your problem gets resolved as soon as possible.

If there are no further questions to ask I would greatly appreciate both a high star rating as well as some written feedback. I would also ask that you closed Aquarian on your end if there are no further questions so that this transaction can be archived for future reference by colleagues treating patients with similar complaints.
