What Causes Pain In Left Buttock While Walking?

Question: I'm having left buttock pain for three months
Pain is highly noted in the early morning and also after walking
MRI shows significant bone marrow edema in left side sacroiliac joint and also in right side but it is very mild. No other bone is involved. Spine is normal.
HLAB27 is positive
Bone scan is normal
Xray is normal
CEO is 4.8
Age 32

Similar pain was noted 3 yrs back but just for 2 weeks.

We call this disease axial spondyloarthropathy(SPA).
ankylosing spondylitis is a prototype disease of this group of disorders.
It does not really matter what you call it, but you definitely have SPA.
Also it needs proper management so that the disease does not damage the spine.
In the initial stages we do rely on painkillers because they give very good relief from symptoms and also stop the progression of this disease.
Once you are better you could make this medicine sos(take it when there is pain).
Also a very important part of the treatment is exercises for the back and general aerobic exercises which you could learn from a good physiotherapist.

Lastly there are some medicines available called biologics which are targeted medicines for this disease but we generally reserve them for resistant cases.

I dont think there is much to be overtly concerned.
The prognosis is good because you have been detected early, most people dont get a proper diagnosis for years.
Secondly at present there is no damage to your spine.
Thirdly such good treatment is now available and under proper guidance I think you should do well.
The spine generally does get affected but there should not be any disability because of the same.

Scientifically there is no specific diet, however eating a balanced healthy diet, helps.
- Maintain your ideal body weight, take calcium rich foods.
- Exercise regularly to mantain flexibility of the spine and also do aerobic exercises.

Pain killers may have to be taken as long as pain and inflammation is present. Subsequently they may be tapered and made sos. However they may need to be restarted in case the inflammation reoccurs.
This disease tends to wax and wane. So your doctor will know when to taper it and when to restart it.
