What Causes Pain In Left Testicle With Normal Semen Analysis?
I am 28 year old male.
I had pain in my left testis last December so after consulting an urologist I went for a scan of scrotum.
Report suggests that I have mild to moderate left sided varicocele and mild right sided varicocele.
Doctor didn’t prescribe any medicine and said if the pain persists, I will have to go for a surgery.
After 4-5 days pain disappeared without any medicine.
Now from last week I have started feeling pain in the left testis again at the bottom which goes till lower left abdomen. It usually happens when I sit continuously for long time (my job requires so). Sometimes this pain become very discomforting but when I stand up and walk for some time it subsides. My left testis hangs lower than right one and I don’t feel any pain while urinating or ejaculating.
I am getting married next month so I don’t have much time for surgery and of course I am afraid too of surgery. I am attaching my scan report of scrotum and semen analysis, kindly suggest some medicine to get comfort from pain and how long I can avoid surgery and also will this condition affect my married life?

Left testis is usually at lower level as compared to right in majority of cases. Your semen analysis report is normal and there is no cause for concern.
Following are the indication for which surgery is usually advised
1. Infertility documented by low sperm count /decreased motility of sperms
2. Reduced testicular volume , decrease in size of testis
3. Intractable pain or symptom not controlled
As this is second time you are having pain , with semen analysis report normal with Varicocele present only on vasalva maneuver (pprocedure which increases intrabdominal pressure ) , I am of the opinion that it can be managed conservatively with Surgery being withheld. Do not worry it will not affect your married life . You should definitely be on follow up with your Physician.
Inadequate support of testis is important factor in causation of pain in testis. It usually occurs by wearing underwear which is too loose or too tight which causes discomfort.
I would suggest you the following which will be helpful I'm relieving pain
1. Adequate support to testis : Wearing the right size of underwear will help relieve pain. Pain due to Varicocele occurs usually on prolonged standing which causes congestion due to gravity. In your case as the pain is occurring during sitting, it's likely to be due to underwear which is too tight.
2. Analgesic like Ibuprofen will be helpful in relieving pain.
3. Avoid hot water bath
