Does he have increased sensitivity to bright light and is uncomfortable to viewing bright objects ?
Does he have radiating pain from the neck to the head and shoulders ?
Has he been sleeping well lately ?
Does he have nausea or vomiting, any visual disturbance ?
From the history, you have provided, it appears that he could be suffering from Cervical spondylosis, Migraine headache or dehydration and chronic fatigue.
Cervical spondylosis could cause a stiff neck, radiating pain to the back of the head and shoulders and sometimes tingling and numbness over the hands. A migraine headache could start at the back and sides of the head and can cause blurring of vision, sensitivity to light and nausea.
Since he is exposed to smoke and dust he could also be having Carbon monoxide toxicity. This causes severe headache, muscle weakness and fatigue.
I would advice you to request him to get some fresh air, avoid bending the neck forwards. If the neck pain has been there for a long time and he is having tingling and numbness of the hands, it is best to get an Orthopaedic consult for a thorough physical examination and proper diagnosis.
Make sure he is well hydrated with water and also electrolyte drinks like Lemonade or Gatorade as electrolyte deficiency due to prolonged sweating could cause most of the same symptoms. He needs to have 6 to 8 glasses of water a day, 6-8 hours of sound sleep. Stay stress free with deep breathing exercises and Yoga.
