I have gone through your query. TMJ disorder can be disc displacement with or without reduction or subluxation or retrodiscitis. If i am your treating doctor I would have suggested you to take an MRI of the joint to see the condition of the disc.
Before starting the treatment we have to remove the cause of the joint disorder, that could be maloclusion, grinding teeth during night time, trauma. Stress plays a major role in this because stress aggravates the joint disorder. Start exercise, yoga and mediation to combat strss. If you have the habit bruxism you can use soft splints after consulting the oral physician.
If there is a disc displacement then I would have prescribed tablet piroxicam 20 mg twice daily on first day and once daily for next six days(if you are not allergic) and advised not to open your mouth too wide and take soft diet. you can also give hot fomentation over the joint. If this treatment doesn't work then I would have suggested the joint lavage with saline or blood (orthocentesis).
You need not suffer as you are currently. A well planned treatment can help you from this situation. It all starts first with MRI of the joint. Visit an oral surgeon and discuss about.
