Brief Answer:
Typhlitis - better to get clinical evaluation, examination, investigations

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Female/75 - pain in right lower belly since 6 am - took 2 Aleve at noon - not appendicitis - has this once a year before which resolved after all night pain.
If there is no associated nausea, vomiting, bloating, loose stools or constipation and fever or chills, the cause is usually typhlitis, that is inflammation of cecum, the starting point of large bowel.
In such a case, anti-spasmodic can be more useful.

Please give additional information about any associated other symptoms as mentioned above and:
Urinary symptoms.
Reference to the back or other areas.
Any relieving or enhancing factors like food, position that you take.
What is the consistency, color, smell and composition of stool?

This feedback will help me further to assist you better.

Since the similar pain was there 1 year back and if you are still in pain please do the following:
Consult your Doctor or ER immediately as it helps for clinical evaluation, examination and investigations to know the exact cause so that a proper treatment can be instituted, and hidden disease or disorder noted.

I hope this answer helps you, please give feedback .

Dear Doctor, thank you for your reply. I have no other symptoms or complaints as mentioned above. The pain is less often now, still comes quite sharply , and the area is quite tender, lower right side and a bit around the back.
I'm going to bed and hope to sleep the night through. Surely this will go away by morning.
I have noted that in between that last attack and this one, the area will often be tender -- just like it is letting me know "it is there" for a short while.
Thank You.
Sincerely, XXXX

Brief Answer:
Further evaluation as advised

Detailed Answer:
Thanks for your feedback.
What worries me is : your history of > in between that last attack and this one, the area will often be tender -- just like it is letting me know "it is there" for a short while.
I would sincerely advise you to please go for clinical evaluation, actual physical examination, preferably by a General Surgeon to get a probable clinical diagnosis to be followed by Contrast enhanced CT scan to get most probable cause.
Also needed are the tests for stool, urine and blood to supplement diagnosis.

I hope to see the diagnosis sooner.
Take care.
With regards.
I shall be happy to see the reports asap.

Dear Doctor, I slept pretty well last night, and now am home from work today.
The acute pain is gone -- now just tender, with little jabs to remind me.
Both attacks have ended after 12-14 hours of acute pain.
I do have an appointment in 2 weeks for a regular check up with my gastro Doc. I am sure she will follow through.
Thank you very much for "talking me through" this time of distress.
Sincerely,
XXXX
