Question: Pain in muscle under left scapula when I breathe also painful to touch. I haven't had an injury.

May be due to nerve pain or referred pain from lung/pleura.

Hello

I have studied your case.

Pain in scapular area without injury may be due to nerve pain or referred pain from lung/pleura.

Nerve compression in spine in lower cervical or thoracic region may lead to pain in scapular area.

Clinical examination will help to know pattern and origin of pain so you can consult orthopaedic doctor or MD internal medicine for this.

You may need investigate like MRI spine, EMG-NCV [nerve conduction study].

As per your symptoms another possibility of pleural or lung involvement which can be pleural irritation/infection.
Your doctor may advise you to do PFT [pulmonary test] and further investigation like X ray chest if required.

Till time you can take analgesic and neurotropic medication like methylcobalamine.
Physiotherapy like ultrasound and IFT [interferential therapy] may help.
