2)Is the pain more on left buttock or right buttock?
3)Is bilateral neuropathy proven on testing(Nerve conduction studies)?
4)Do you have any complaints in lower limbs?tingling,numbness or weakness in lower limbs?
5)Do you tend to have morning stiffness?If yes,for how much duration?Are movements at the hip joint painful?

Lumbosacral radiculopathy.

Thanks for being in follow-up and providing further details.
Your clinical details of pain more prominent on right buttock along with numbness, is likely pointing towards lumbosacral radiculopathy as the cause.This may be present in addition to the neuropathy. Though you mentioned that you had a degenerative spine on MRI one year back but generalized degeneration does not precisely says for anatomical correlation of the pain.There must be some canal stenosis or forminal narrowing at the L-S spine region for the current symptoms.At this time,I suggest you to undergo a fresh MRI of L-S spine to look for the same.Once the extent of pathology is localized,we can plan the intervention for the pain.Epidural injections may be tried for response though you mentioned that one cortisone injection made a little difference. Depending on the extent of the pathology,it can be decided whether you are a candidate for surgery or minimal interventions will suffice.
