What Causes Pain In Right Hip While Standing And Walking?

I am writing regarding my father. He is 74 years old. He is suffering from pain in the right hip, center of the lower back and groin since a week. However this pain is there only while standing and walking. He has no pain while sitting, getting up from chair or while lying down. He did not have any fall or injury.
He showed to a doctor who prescribed the following :
NISE -MR ( 2 times a day), Ravohex 20 (once before breakfast), Bio D3 strong (1 month). He has been taking since Sunday morning ( 2.5 days) but there is no reduction in the pain even temporarily.
Can you please advise the cause of right hip, center of low back and groin and recommend next steps.
Osteoarthritis of right hip joint.
Detailed Answer:
Hi XXXXXXX
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:
1. He might be having osteoarthritis of the right hip joint. This is causing pain in the right hip. The pain is due to rubbing of the bones against each other during movement. This pain is radiating to the groin. If it progresses, pain can occur during rest too.
2. To reduce his present pain, he is having an abnormal posture during walking, which is affecting his back, resulting in lower back pain. Proper posture, and mild back exercises should reduce pain.
3. X-ray of the right hip is essential to look for the amount of damage.
4. Ibuprofen or Celecoxib for pain twice daily should be helpful. Bio D3 can be continued.
5. Other methods useful are
a. weight reduction
b. Walking stick
c. avoid activities increasing pain and choose comfortable position.
d. Maintain proper posture. Improper posture causes further damage.
After X-ray, based on findings, if necessary, corticosteroids injection to joints can be taken.
An Orthopedician or an hip specialist can be more helpful in management based on amount of damage. Physiotherapy will be essential.
Hope it helps.

My father is already taking the following medicines
NISE MR (twice daily) for 3.5 days but is not having any reduction in pain.
He had taken combiflam prior to starting NISE (MR) but there was no reduction in pain.
Why is it so?
Also his current height is 5 ft 6 in - reduced with age(it was 5 feet 8 inches earlier)
and his weight has been stable around 75 Kg for several years.
Can you also recommend some age specific exercises for pain reduction keeping in mind that does not do any exercise except for regular long evening walk.
Also he has varicose veins problem in left leg below the knee for around 20 years and takes BP medicines Minipress XL 2.5 once daily for around 15 years.
Weight loss and non-weight bearing exercises.

Hi XXXXXXX
Thanks for asking again.
The rubbing of bones, occurs only during movement. As the soft tissue inflammation is not involved much (if involved, would have pain during rest too), NISE MR or combiflam won't be much helpful. If the pain is at rest, it will get completely resolved with above painkillers.
Another 8-10 kg weight loss would be beneficial. With age, there is muscle loss and mostly fat tissue and bone. Weight reduction through diet and mild exercises will be helpful.
Non-weight bearing exercises for joint to improve muscle mass. As his right leg is involved, left leg stretching slowly front and back. Avoid using XXXXXXX type of toilet or any weighting exercises involving the right joint. Walking, only with walking stick is advisable. Cycling - specifically with stationary cycle, if not painful can be tried.
Varicose veins can be controlled with compressed bandages. Need to continue with regular blood pressure medications.
Investigations, as discussed earlier is necessary to identify the amount of damage.
Hope it helps.
