I will like to see a complete and comprehensive clinical examination and basic testing including Imaging studies to identify the problem and propose solutions. If this is beyond his competency, then a referral would be issued to the right specialists. For now, pain medications like Ibuprofen and Tylenol and postures/positions that relief pain is highly encouraged while we work on identifying the exact cause and providing long lasting solutions.
In all, start with evaluation by your PCP. Should you not want to, then a neurosurgeon is the best single specialist to start with as this seems to be related to the nerves. Tests would mainly include imaging studies and further testings would be guided by preliminary findings.

It is okay to try and locate the exact nerve damage using pins before an MRI. MRI looks for problems with the bone and soft tissues and cannot locate nerve problems. If the Xray had been suggestive of a possible bone lesion, he would have asked for more informative imaging. Probably, there were no arguments in support of this and the reason why he is not pursuing in this direction.
In all, the preliminary findings do not seem to be suggestive of structural abnormalities and further detailed imaging may not help in this light. I have no worries getting the nerve being affected mapped out with pins. Feel comfortable too and follow through hoping we finally get answers to address your concerns and relief the pains.
