What Causes Pain, Swelling And Redness On Abdomen Post Cesarean Delivery?

Question: I had undergone a cesarean delivery on 17:11:14. After that from 3rd day she is feeling a constant pain in her abdomen, coupled. Earlier, when contacted to consultant gynaecologist, she said its normal and part of healing. But the problem persisted and grew further, coupled with swelling and redness of marks on abdomen. The problem effected the daily routine of my wife, even so she could not sit, and move without help.We done MRI scan on 12:01:15 which revealed "edema on scar site/ normal bowel movement".A treatment was run for a week at hospital including antibiotic dose of pipzo, amikacin through iv, which did give the relax, but the problem again resurfaces after one week.We searched on internet about the problem and got information about abrasions during surgery, which requires another surgery to treat it.

What kind of MRI was done? Was it enterography MRI (was she drinking a contrast right before the test) or regular MRI? Where is the pain located? Is the pain cramping? Does she have any diarrhea, constipation, gas obstruction? Is there any lump in the scar area? Is she able to pinpoint the exact area of pain? Does the pain get worse during her period?

Any abdominal surgery can cause adhesions in abdominal cavity. Adhesions are a kind of tight ribbons hanging inside the abdomen. Such ribbons can make intestines narrow or twisted and give pain and occasional gas obstruction.
Typical symptoms of such problem are cramping, bloating, gas obstruction and diarrhea which gives pain relief.

Endometriosis is the other possibility which can give mentioned symptoms. Uterine surgery can cause uterine cells implantation in the abdominal cavity. It can give pain during intercourse, abdominal pain, ample periods.

I suggest her to have enterography MRI if symptoms seem to be caused by adhesions. If nothing will be found, diagnostic laparoscopy can be the best option.

it was a simple mri scan done without contrast

my wife is asthmatic and allergic , will mri with contrast will cause any allergic reactions, as she felt extreme cold and allergic itching when one ct chest contrast was done couple of years back.will mri with contrast show the adhesions, infectective tissue, nerve entrapment
or blood vessels damamge or cloting

can we go for usg colour dopller or abdominal x ray first to know the intestinal obstruction

yes she caninpoint the pain which is specifically above the scar site in rightloweer abdominal quarter she faces d8fficulty while passing gas and stool.she is on rabicip l and naturolax now a days she had periods four days back and there were cramps too

is it safe tohave mri after periods will it affect the mri reports. also she is having headache tremors palpitation and breathing problem also with a generalised feeling of not well being will it be safe to have an mri contrast keeping in voew of her overalk XXXXXXX .pls advise asap

MRI contrast is safe and do not cause allergy.

MRI contrast is safe. Even in case of CT contrast allergy, MRI contrast do not cause such problems. So she may have it.
MRI enterography should detect partial bowels obstruction due to adhesions. Infection, nerve entrapment or blood clots are not the case for sure. Symptoms of this issues are much different.
Ultrasound is not a good idea. This test is less accurate than MRI. Abdominal X-ray can be done to detect bowels obstruction. But it may be negative in case of partial obstruction.

Sir, I have gone for ultrasound and it's report is enclosed

It shows some finding of tuberculosis, I am also enclosing image of usg too..

Tuberculosis sounds really hardly possible.

Turberculosis of this are wound be an extreme rare. So rare that it sounds almost impossible. Abdominal turberculosis gives enlarged lymph nodes and fluid in abdominal cavity. So I really do not think she may have tuberculosis in that area.
MRI, which is much more precise test would be able too see it as well. As it is under the skin, biopsy should be done.

Sir had to go a new usg tday as doc wasnt convinced with the findings..enclosing the report ofusg again pls guide for further proceeding ..will mri help in further evaluation or ctscan for better finding..

Ultrasound is really not a good way to diagnose her problem.

USG is very sketchy diagnostic method. It really does not give whole image of abdominal cavity and especially is not good in case of intestine narrowing/ adhesions. Her problems seem to be caused by adhesions.
The fluid collection which has been found in the ultrasound is nothing important and for sure does not cause any pain.
Enetrography MRI or CT enteroclysis should be done. MRI has non allergic contrast so is better at her case.

thanks doc what does that 3 cm thing indicates in usg ,.is that a indication of dead tissue or necrosisi and also who is the bestperson to deal this ..is it gynae ,general surgeon or physician or someone else

read on net about necrosis and find that extremely dangerous..does my wife have that??

This 3 cm finding is a fluid collection, not necrosis.

No, its not a dead tissue. This is a fluid collection. Its really a thin layer of fluid. It can be left alone. Such fluids collections can be seen after surgeries. So this is nothing she should be afraid of.
So once again, its not necrosis. Necrosis would give swelling and fever.
The best doctor she should consult is general surgeon.

I have got that stool test done reports are still awaited..The surgeon whom I was consulting has prescribed me a chest x ray and tb gold test..x ray chest came normal and tb gold test was also negative.got the mri done today ..I m enclosing the teports received so far..pls guide..I went to my gynae also.she has prescibed me sumag ointment twice a day

Medicines prescribed by gynecologist wont help to solve the problem.

Tuberculosis was a very impossible idea and it has just been confirmed to be negative.
Please believe me, there is no infection. Not at all. There is no sign of infection in your blood work, no sign of infection in ultrasound. Just a small fluid collection. This is not serious and can be left alone. Necrosis is not the case for sure.
Necrotic tissues get infected very quickly. This gives massive inflammation with swelling, redness, high fever and pain.
Medicines given by gynecologist wont solve your problem. Zinase which is a pain killer can help a little with the pain.
Your main problem seems to be bowels and adhesions conflict causing partial bowels obstruction.
Once again, MRI enterography (not just a regular MRI) or CT enteroclysis can detect it.

sir are these test same conducted for abdominal tb.if not what can be done to find if i have any any kind of abdominal tb...does that comes in these blood reports

yesterday she accidentaaly xonsumed very fine particles of glass while having milk feom a glass bottle.will it affect her..how to get rid from them feom the body

what is this soft tissue track in my mri report..and hypoechoic collectionnof 3 cm..is this some sort of dead tissue..what problems will thos dead issue cause in coming future.

what is this sumag ointment used for will it be safe for her...

Tuberculosis is ruled out.

Detailed Answer:
Attached tests results rule tuberculosis out, no matter where is it located. Please believe me, you do not have tuberculosis. It is very rare in abdominal area and do not give just a solitaire lesion seen in ultrasound.
So forget about tuberculosis.

Small pieces of glass wont make any harm in her body. Only a large part can do that and stuck in digestive tract. Small parts will be excreted with stool.

Like I have said before, hypoechoic 3 cm collection is a FLUID. Not necrosis, neither infection. Just a fluid. It does not cause any problems.

Adhesions can be diagnosed with MRI enterography.

Sumag ointment is for skin protection. It has some anti-inflammatory effect. Can be safely used with her medicines.
