What Causes Painful Small Brown Scab On Buttock?
Question: this started as a single small brown scab but over months has changed colour, texture and size. My partner is complaining that it is now stinging and feels quite sore if touched or if hot water lands on it during a shower. He is 44yo with no known allergies

Hi Dr Savaskar M.N. Greatly appreciate your prompt reply. The picture is of the rash/scab is on my parter's left buttock. He claims he thought it was a pimple about 12 months ago but it's been progressively getting worse. The last couple of weeks especially, it's really flared up. He has not used any creams or antiobiotics etc to treat it and has been too embarrassed to see his doctor. He is now complaining of stinging pain which he didn't previously have. The marking started as a pimple-like bump which he scratched and then it scabbed. It's now red and the surface of it is risen and has yellow crusty bits (which I assume are puss). As mentioned, he's now had this for over 12 months but the last couple of weeks it's really changed dramatically and I'm extremely concerned

Hi,Thanks for the updates with appreciation of my last reply.
I reviewed the photograph and your query.
The health concerns of your partner are due to the recurring folliculitis of the buttocks.
Your partner seems to have superficial and low grade infection of the buttock hair follicles and is locally controlled due to good body immunity.
Lately stinging/burning pain is due to the blistered rash being opened up and
due to the acute superficial inflammation the hot water gives stinging or burning pain / and or pain from the shower due to the falling water on the inflamed hair follicle with furunculitis.
Recurrence and Chronic nature of this blistered lesions is due to its location near the rear end which is highly soiled by the unwanted bacterias
Advise suggested-
I would advise you to consult ER primary doctor, who would treat it as follows-
Regular Hygienic cleaning of the buttock with betadine lotion
Rest to the parts
FBS And PPS as your partner is 44 yrs old,diabetes needs to be ruled as a factor causing recurrent and chronic folliculitis of the buttock are.
Antibiotic (Such as Floxin) with anti-inflammatory Tabs(as Voltaren) x for 5 days
If no relief of the condition, pus culture to rule out Pseudomonas folliculitis which causes Chronic Folliculitis mostly.
Avoiding friction and rubbing of the buttocks by reducing frequency of driving and cycling and travelling.
Hope this would resolve the chronic complicated issue of the buttock follicullitis.
