What Causes Painful Urination Along With Chills And Fever?

I read your question and i understand your concern. Painful urination with hesitancy and rigors and chills all point towards two things. A urinary tract infection and / or infection / enlargement of prostate.
You need to get a urine routine exam and a prostate check. If BPH you need to be started on prostate meds those will relieve the symptoms. If UTI found that will be prescribed with antibiotics.
Headache may be a concomitant finding or a sequelae of nifedipine you have been using as antihypertensive. Take some acetaminophen or ibuprofen for that.
