What Causes Persistent Abdominal Pain In The Mornings?

Question: My wife 49 yrs old is having pain in abdomen. It is more severe in early morning. Dr has given spasmelan tablet with Mac rabonik and mebex 100. Most of the blood and sonography reports are negative. Her esr is 40 and widal test is showing 1: 40 in H and O. Please inform what could be the reason for her continued pain in abdomen. She weighs 59 kgs down from 62 kgs in 10 days.

Thanks for your query.

Read and understood the history of your wife.
To recapitulate: Female/49- lost 3 kg in 10 days - pain in abdomen more in the early morning - under treatment - Sonogram normal - blood shows ESR of 40 and Widal is 1:40

I would like to know a few details please:
-Is there any bloating, nausea, vomiting, loose stools or constipation?
- Is there fever or chills?
- Since when is she having this problem?
- Any other relevant history?

The history is suggestive that the pain may be due to the following reasons:
- Widal of 1:40 is taken as negative hence may not be typhoid.
- Increased ESR of 40 and weight loss indicate a chronic problem. This may be due to some intestinal problems and can be diagnosed by few investigations Like:
-Enteroclysis under fluoroscope control so that the Radiologist is in a position to find the problem.
-CT scan of the abdomen.
-X-ray of the chest
-If the problem is long standing, diagnostic laparoscopy helps in some cases.

No Sir the problem is recent and not chronic. .she doesn't suffer from nausea fever or vomiting. The pain is only early morning. Dr has given cyclopam ofloxacin and zentel tabs. Dr has asked to visit him after 5 days. Is ct scan necessary and it is to diagnose what. ?

I though as the ESR is high and when has lost weight it is possible that there may be an old problem too. CT scan may be necessary only if there is no relief.
Ct scan helps to detect many diseases that the ultrasonography can not show like appendicitis, bowel (intestinal) wall edema. Lymph nodes and so on. Ultrasonography is a preliminary investigations.

Noted that there is no nausea, vomiting or fever.
Let us see the results and decode.

May I please know where exactly is the pain? is it limited to one part only or all over the abdomen?
Is the stool and urine normal?

Thank you Doctor for your reply. The pain starts from upper right side of abdomen in morning and then in other parts .If it is intestinal problem My worry is can the ailment be cancerous. Pl advice.
