What Causes Pink Vaginal Spotting While Having Ovarian Cyst?

Question: hi , i've been having slight pink spotting about 1 1/2 weeks after my period. i spoke with my doctor and he ordered a pelvic ultrasound internal and external. all normal, i have a small cyst on my left ovary, clear fluid and a stage one drop in my uterus as stated in report. i had digital exam by my doctor and he also stated the drop uterus stage one. i would only have very very slight pick discharge maybe every 2 days at the end of the day. this morning after slight for play i experienced a bloody discharge. i have never experienced any pain, or crapping. i had blood test drawn all normal. i have notice some dryness. . he had told me that i would need to be spot free for at least 21 days. i have always had normal paps and negative hpv test.
he had mentioned that i may need a d&c, but i don't have heavy bleeding. about 2 months ago had passed some clots but other than that nothing. please any advise

UTERUS
An anteverted uterus is visualized measuring 96 x 52 x 57mm.

The endometrial lining measures 9.3mm.

OVARIES
The right ovary is not visualized.

The left ovary measures 54 x 33 x 33mm. Within the ovary which comprises
most of the ovary is a clear cyst that measures 36 x 27 x 30mm. Doppler
velocimetry of the cyst wall shows normal resistance.

CUL-DE-SAC
There is no free fluid visualized in the cul-de-sac.

I have gone through your query and being your physician I completely understand your health concerns..
It is not normal to have intermenstrual bleeding at the age of 51. Since with ultrasound other causes have been ruled out like fibroids , adenomyosis etc. Also your pap smear and HPV testing appear normal.
So the only option left in order to reach the cause of bleeding is to biopsy the endometrium ( inner lining of uterus) through D&C. It appear quite logical in this setting and I will recommend it for you too. Also endometrial thickness had increased on ultrasound scan.
Cyst in ovary has nothing to do with bleeding and should not bother you much.
Follow your doctor instructions.

I really appreciate your follow up....

Normally in every menstrual cycle , under the influence of estrogen and progesterone endometrium thickens. And at end , it start shedding in form of menses. This is normal in child bearing age.

After 50 years of age or menopause since estrogen and progesterone levels decreases and stimulus for endometrium to thicken ceases. If one finds thickened endometrium after it, it raises suspicion of any abnormal development including hyperplasia and malignancy. So endometrial biopsy is indicated to look for abnormal changes in cells so that timely intervention can be done.
