This can happen to anyone. This phenomenon, commonly called "hearing oneself's heart beat", is most commonly caused by extensive accumulation of wax in the respective ear. This causes a person to be stressed from this phenomenon, which further produces more forceful heart beats, which are heard especially in the accumulation of the wax in the eardrum.
Therefore, my first recommendation is that you visit a primary healthcare unit, where a nurse will perform cleaning of the wax attached to the eardrum. My opinion is reinforced even more from the fact that, by switching position, there is also a change in this phenomenon.
However, if even after you have done this, you continue to selectively hear your own heart beat in one specific ear, an Echo Doppler of the carotid (neck) arteries, is recommended.
