Question: Yes, please, thank you. 68 y/o caucasian female, excellent health. Only medication is Premarin .3mg Premarin and Prevacid and I take multi-vitamin. Have been under tremendous stress for past month and over the last 4 days have had quivering sensation with flip flop sensation in left chest close to midsternum, no chest pain. When this happens I get a cold sensation in throat with impulse to cough. I've taken my BP and P several times and normal range 132/78, P 75, 130/75, P72 etc. Had follow-up post-op visit today with my GYN for A & P repair 8 wks ago. Told him of sensation, very concerned and wrote order for me to have cardiac study. I am traveling (6+ hrs) from home. Should I be concerned enough to seek care w/out waiting to get home?

Brief Answer:
Cardiovascular involvement to be ruled out

Detailed Answer:
Hello dear,

Thanks for your query.

You have not mentioned regarding any history of injury/ trauma to the chest.
Under such circumstances and based on the case history as mentioned in your post, the symptoms can be attributed to:

1. Cardiovascular cause: as suspected by your treating Gynaecologist.
Though the recordings of BP and P are within normal limits and you have not mentioned about any past history of Hypertension, Lipid disorders, Diabetes, etc. basic cardiac studies need to be performed to rule out any Cardiovascular involvement for your symptoms.

2. Gastrointestinal cause/ Acid reflux: As suggested by the cold sensation in throat with impulse to cough.
Kindly continue the Prevacid as advised by your Doctor. It prevents acid formation in stomach. You can also use Domperidone preparations. This strengthens the lower oesophageal sphincter and prevents acid reflux.

3. Anxiety with rapid breathing: Since you have mentioned in your post that you had been under tremendous stress for past month, the role of stress & anxiety can also be attributed for your symptoms. Anxiety further acts as a risk factor for Cardiac diseases.

So, in my opinion, it will be better to undergo the cardiac evaluation.
This will be helpful in diagnosing your condition better & take appropriate management steps.

There is no need to worry, you will be fine.
Wishing you a good health.
Take care.
